About Lisa Hoehn

Lisa Hoehn is a nurse practitioner at The Kidney Experts. She joined the practice in January 2021 and works with patients living with chronic kidney disease across our West Tennessee clinics.

Her public role focuses on nephrology education: helping patients understand kidney lab trends, urine testing and the questions worth bringing to a visit. Kidney disease is easier to navigate when the care plan is explained clearly, and that understanding may take more than one conversation.

Ongoing CKD management often includes follow-up between major decisions, with attention to how the plan is working and what questions should come back to the care team.

What this page includes

This profile is limited to the practice information we publish about our own team. It is not a directory record, and it is not medical advice about your own care.

What this care looks like

The work below is the day-to-day of the practice. What your own visits cover depends on your labs, your history and what you and your care team decide together.

  • Ongoing chronic kidney disease management and follow-up visits
  • Patient education about labs, medications and daily routines
  • Practical questions about diet, fluids and over-the-counter medicines
  • Keeping care plans on track between nephrology appointments

You will meet more than one of us

Kidney care runs on continuity. Our clinicians and clinical staff coordinate follow-up across visits so the care plan stays connected.
