Being a new patient at a specialty clinic can be overwhelming. We strive to make all patients feel at home throughout their entire journey with us.

To save time on your first visit, fill our new patient registration form before your visit. Our staff will receive a copy when you submit it.

Before every visit with The Kidney Experts, you will need to get new labs drawn 1 week before your office visit. This allows our doctors to assess your kidney function.

Please remember to bring your current medications to every visit. We want to keep your records 100% accurate. This includes having your correct medications in our system. More than anything else, it affects the quality of care we can provide you.

Consent for Payment

I hereby authorize payment of medical benefits billed to my insurance by The Kidney Experts, PLLC Physicians. I have listed all health insurance plans from which I may receive benefits. I hereby accept responsibility for payment for any service(s) provided to me that is not covered by my insurance. I agree to pay all copayments, coinsurance, and deductibles at the time services rendered. I also accept responsibility for fees that exceed the payment made by my insurance, if The Kidney Experts, PLLC Physicians do not participate in my insurance. I hereby authorize The Kidney Experts, PLLC Physicians to use and/ or disclose my health information which specifically identifies me or which can reasonably be used to identify me to carry out my treatment, payment, and healthcare operations. I understand that while this consent is voluntary, if I refuse to sign this consent, The Kidney Experts, PLLC Physicians can refuse to treat me. I understand this authorization can only be revoked in writing. If I revoke my consent, such revocation will not affect any actions that The Kidney Experts, PLLC Physicians took before receiving my revocation.
