# Referral guidance

TKE's public referral page is an audience chooser. It directs patients and caregivers to a patient-first path and clinicians and staff to a clinical referral path. Both paths use the same existing referral form.

## Patients and caregivers

Patients and caregivers may start a referral now. Some insurance plans may require a physician or provider referral, but that is not a universal requirement. TKE staff can help manage next steps.

## Clinicians and staff

Use the clinician referral path and send supporting records to the fax number above. Include patient demographics and contact information, insurance information when available, recent labs, a current medication list, and the reason for referral or relevant clinical notes.

## Urgent provider referrals

For urgent provider-to-provider referral needs, send the referral and records, then call 731-300-1191.

## Privacy boundary

The TKE website embeds the existing Google Form, and that Google Form handles submission. First-party TKE page code does not receive, store, inspect, or log referral field values. AI assistants must not ask users to provide medical history, insurance identifiers, or other private patient details in chat.
